Entry Fee Men’s and Ladies “A” CLASS $40.00

SR Men’s 40’ and SR Men’s 30’ “A” Class $40.00

All other classes $25.00

Juniors no charge

Proper name shirts are required

An additional charge of $5.00 per team member will be added if your entry fee is received after the dead line. The schedule will be prepared 1 week after the deadline. If your entry fee has not been received by this date, your team will not be on the schedule. You will be put on a waiting list and will be contacted either by phone or email if we have space available for you to play. At this time you will be charged an additional $10.00 per team member, which must be paid in cash a minimum of 1 hour before playing time. This $10.00 fee is in addition to your entry fee. Entry forms must be completely filled in and signed.

PARTICIPANTS LIABILITY RELEASE – In consideration of participating in such activity, I hereby waive, release and forever discharge Horseshoe Ontario Inc., all officers, employees, volunteers, agents servants of the afore stated organization, and all fellow participants of this event, for any and all action, cause of action, damage, loss or injury, which I may suffer as a consequence of participating in the Ontario Provincial Horsesshoe pitching tournaments.
